Definition Diffusion-weighted whole-body MRI and the so-called axial MRI (evaluation of the spine and the pelvi-femoral area only) are more sensitive than bone scan and targeted radiographs and seem equally effective as 11C-Choline PET/CT in detecting bone metastases in patients with high-risk PCa. Their sensitivity for lymph node metastases remains low, even if it is slightly higher than that of 11C-Choline PET/CT in high-risk patients. However, little is known regarding the accuracy of whole-body or axial MRI in patients with biochemical failure after RP or radiation therapy. Therefore, the role of these techniques in detecting occult bone or lymph node metastases in the case of biochemical failure remains to be assessed.
